Request for Accreditation Services

If you complete this form online it will automatically be submitted to the appropriate Committee on Accreditation. As another option, you may also create a printable version to complete and mail.

There are three (3) options for submitting a Request for Accreditation Services Form to CAAHEP.

Option 1. Preferred. You may complete the form online and submit it electronically, directly to the appropriate Committee on Accreditation. When you submit the application electronically you will automatically receive a copy of your completed application via email and you have the option of printing a completed copy of the form once you are finished with the application.

Option 2. You may complete the form on-line but print it and send it to the appropriate Committee on Accreditation via regular mail.

Option 3. You may print a blank copy of the form, complete it by "hand," get the required signatures and then mail it to the appropriate Committee on Accreditation.
