Request for Accreditation Services

There are three (3) options for submitting a Request for Accreditation Services Form to CAAHEP.

Option 1. Preferred. You may complete the form online and submit it electronically, directly to the appropriate Committee on Accreditation. When you submit the form electronically you will automatically receive a copy of your completed form via email and you have the option of printing a completed copy of the form once you are finished with the form.

Option 2. You may complete the form on-line but print it and send it to the appropriate Committee on Accreditation via regular mail.

Option 3.You may complete the form on-line but print it and fax it to the appropriate Committee on Accreditation.
